Role Introduction

•    We are looking for a B3 Weight Management Facilitator who is passionate about supporting the delivery of an effective, responsive and caring service.

This post is permanent, flexible (hybrid working), part time 18.75 hours. Our service run’s 8:30-4:30 Monday to Friday. 

Leeds Tier 3 Weight Management Service is a specialist service offering support for adults with severe and complex obesity offer a 12-month programme led by a multi-disciplinary team of experienced healthcare professionals, including:

•    consultant physician (who specialises in obesity medicine)

•    physiotherapist

•    dietitians

•    mental health specialists

During the programme, the team support patients make sustainable diet and life changes to improve your health.

Duties relating to your role include:

•    MDT working 

•    Home visits for patients that are housebound

•    Metric Clinics to support Service Key performance indicators 

•    Supporting staff with resource/leaflet development 

•    Supporting the team with student training

All staff are provided with a laptop and smart phone for hybrid working and remote access to SystmOne. 

You will work with adults who are living with complex obesity BMI’s of 40 and above. The list below provides a detailed overview of your daily/monthly work activities:

1.    You will work as part of multidisciplinary team providing using SystmOne to communicate with our wider MDT.  

2.    You will be involved in obtaining user feedback. Examples include collecting Friends & Family Test (FFT’s) to ensure effective patient care and accountability.  

3.    You will offer 1:1 Metric clinics at designated health centres. Within these clinics you will be measuring patients’ height, weight, body composition and blood pressure. At current these are ran at Chapeltown and Parkside Health Centres.   

4.    To provide relevant information regarding the service and our pathways. 

5.    You will be involved in supporting our newly developed GLP-1 medication pathways. 

6.    You will attend relevant Team meetings, and other meetings as necessary to discuss care of patients.

7.    You will occasionally provide home visits for housebound patients following Trust home visit policy.

8.    You will liaise with the Team Leader, Clinical Lead & MDT to provide a comprehensive Weight Management service. 

Skills and Qualities  

1.    Previous experience as a health care assistant within the NHS.  

2.    Experience working in a multidisciplinary team. 

3.    Experience ensuring patients and carers are dealt with in a way that maintains their privacy and dignity and any distressing issues are approached sensitively and professionally. 

4.    Experience or special interest in weight management.  

Full training will be provided to allow you to ensure you are comfortable and confident in the role.
